For each category, record the item, date range, where it currently exists, whether you already possess a lawful copy, original format, and why it may matter.

Suggested categories:

  • paystubs and wage statements;
  • schedules and time records;
  • offer letters and compensation plans;
  • job descriptions;
  • performance reviews and goals;
  • discipline and PIP documents;
  • complaints and acknowledgments;
  • leave/accommodation communications;
  • medical-work restrictions actually provided to the employer;
  • investigation communications;
  • separation notices;
  • unemployment notices;
  • EEOC/NERC or other agency filings;
  • relevant texts, emails, or messages you lawfully possess.

Do not use the worksheet as a reason to take privileged material, trade secrets, other employees' confidential records, or files you are not entitled to access.
